To register your child for a class at CLC, complete the
2008-09 Registration Form
and return it to the CLC office. Please check in advance regarding space
availability.
Once a child's enrollment is confirmed, forms must be completed as follows:
The Enrollment Agreement must be signed by both parents and returned to the
CLC office within 14 days of enrolling.
The following medical, emergency and informational forms are required to be
completed and returned to the CLC office before the first day of class:
Immunization Record (signed and dated by child's physician)
Health Inventory and Addendum (signed and dated by child's physician)
The Medication Order Form is required only for children with allergies or other
medical conditions which require possible use of medication or an action
plan.
